If you mean the Preview app on a Mac, don't ever use it to open PDF forms. It corrupts them quite extensively when it saves them, which can happen automatically (no user confirmation) when the PDF is closed. Preview has lousy support for forms even if it didn't corrupt them. The corruption that Preview can cause does not just affect forms-related things, and includes things such as removal of digital signatures, layers (OCGs), JavaScript, and other things it isn't capable of dealing with.
